    Who we are: Founded in 1985, Sangre de Cristo Hospice & Palliative Care is a local CHAP accredited non-profit healthcare agency specializing in the end-of-life care.  We provide comprehensive, compassionate care to all who need it, regardless of their ability to pay.

    Our hospice team consists of a medical director, nurses, aids, social workers, chaplains, therapists, counselors and volunteers.  They work together with the Primary Care Physician, caregiver(s), and the patient to develop a care plan that focuses on pain and symptom management.

    Our comprehensive array of services include:

        In-home hospice and palliative care
        In-patient hospice care at skilled nursing and long-term care facilities
        Social work services
        Chaplain services
        Volunteer services
        Grief counseling, education and bereavement services
        Caregiver support
        Special recognition of veterans
        The Joni Fair Hospice House, a 10-bed in-patient facility with home-like surroundings
